New Patient Forms »Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid as per the contract agreement. We work with most insurance plans as an out-of-network provider and will do our best to help you file your claim. You will be responsible for any balance not taken care of by your insurance policy. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them. Scissortail Endodontics does not accept or participate in Medicaid. Scissortail Endodontics are in-network ONLY with Delta Dental. Once treatment has started, we will file your claims.
